As a means to improve the collection of field data which is required for decision making, planning, situation analysis and forecasting, FAO in collaboration with affected countries has designed a simple form that should be used during survey and control operations. The two-page form is available as a PDF in English and French. It is compatible with the latest eLocust3 data and supersedes the previous form issued in 2009. If eLocust3 is being used, then there is no need to use this form unless officers wish to have a duplicate (backup) copy of the field data. Updated: Aug 2014.
